Society Report Forms

It is important that incidents that require formal investigation are reported according to Society policy and procedure and correct forms are used.

Red Card reports

 

If you have issued a red card you must contact the Society Secretary.

The completed form should be  sent to the Society Discipline Officer by 4pm the following day at the latest. This allows time for checking and discussion before the 48 hour deadline for submission.

Match Officials  abuse report 
 

All incidents of match official abuse must be reported verbally to the Society Secretary at the earliest opportunity. 

Referees should complete the report form ,save a copy, and send the report to the Society Secretary.

Match abandoned Report

If a match is abandoned the referee must report the incident to the Society Secretary.

Serious injury report
 

If a  player is seriously injured the referee should fill in the online incident report. 

A serious injury is defined as an incident where a player is admitted to hospital and is kept in overnight. It is therefore important that clubs report incidents to referees.
